I. Renewal for Undertakers and Embalmers (CEUE) License)

Department Circular No. 2009-0018: Reiteration on the strict implementation of Committee of Examiners for Undertakers and Embalmers (CEUE) Resolutions regarding Embalming Program.

To ensure the effective, implementation of the Embalmers Program of the Department of Health (DOH) and facilitate immediate response to queries and complaints regarding the practice, the Human Resource Development Units (HRDUs) of Centers for Health Development (CHDs) are designated as Coordinators for Embalmers and Massage Theraphy Programs as per DM No. 2008-0009;

Requirements :

Medical Certificate

PTR (from LGU)

Resident Certificate

Identification Card

CEE Certification of Credits Units Earned (30 units )

Certificate of Registration

1 x 1 ID picture (2 copies)
1 x 1 ID picture (1 copy)
